About the Role

As the Navigation and Community Support Officer, you will play a pivotal role in providing accurate advice and proactive assistance to older individuals and carers, ensuring they navigate the aged care system with ease. Your expertise will drive impactful initiatives and foster effective relationships with service providers, contributing to a meaningful difference in the lives of our community's older adults.

Skills and Experience

  • Hold a tertiary or post-graduate qualification in health, social services, or related field and/or or demonstrate knowledge and experience in providing information, linking, and advocacy for older people.
  • Have extensive experience in the health and aged care service delivery sector, with a deep understanding of the complexities associated with healthy aging, the community sector, and strategic direction.
  • Showcase skills in supporting individuals to comprehend information and options related to the aged care system, as well as in accessing services and supports of their choice.
  • Demonstrated ability to actively contribute to the continuous quality improvement of a service in response to organisational changes, policy contexts, or other practice-related issues.
  • Exhibit effective time management skills, including planning, organising, and prioritising workloads to meet deadlines.
